The Role
Help children and young people with significant disabilities transition into their new program of care.
  
Duties
  • Ensure the client receives high-quality services, care and supports
  • Ensure the client’s physical, emotional, behavioural and social are supported
  • Provide leadership and support to the care team
  • Assist client with daily living such as meals, mobility and hygiene and house
  • Liaise with a multidisciplinary team to care for the client
  • Assist and make suggestions in the planning of ongoing care regimes
  • Assist in the transition of children and young people to their new home by creating a warm and supportive environment
  • Monitor, record and report symptoms or changes in client’s conditions
  • Maintain accurate and detailed client records
  • Create and maintain a positive professional relationship with the client
  • Liaising with our Care Coordinator to ensure effective communication with residents/relatives
What you will need
  • Registered Nurse AHPRA registration
  • Experience managing individuals and teams
  • Experience working with young people with complex needs and disabilities
  • Experience in program management, service development and delivering positive outcomes for young people and families
  • Team management experience
  • Work rights in Australia
  • Current Criminal Record Check (can easily be applied for)
  • Current Working with Children Check (can easily be applied for)
  • Full immunisation history
  • Current First Aid and CPR (if expired, training is available)
  • Current NSW Drivers Licence
  • Ability to work well independently or in a team
  • Excellent communication skills
  • Excellent interpersonal skills
  • Excellent organisational skills
  • Skilled in conflict resolution, change and financial management
  • Ability to assess, adapt and react quickly under pressure
  • Proficient computer skills with Microsoft office
